| Abstract: | Tato práce je zaměřena na antibiotika, konkrétně na karbapenemy a jejich rezis-tenci u Enterobakterií. Snažím se zde krátce popsat historii antibiotik a konkrétně kar-bapenemů, jejich mechanismus účinku, mechanismy rezistence a snažím se zde charakte-rizovat klinicky významné rody čeledi Enterobacteriaceae, konkrétně rod Escherichia coli, Klebsiella spp. a Enterobacter spp. Zvláště se zde věnuji jejich laboratorní diagnos-tice a s ní spojenému vyšetření citlivosti na antibiotika. V praktické části se zabývám analýzou dat z nově zavedené laboratorní detekce karbapenemáz u enterobakterií v Ne-mocnici na Homolce, což bylo hlavním cílem této práce. Na základě cílů jsem si stanovila výzkumné otázky, které se mi povedlo praktickou částí práce zodpovědět. V diskuzi se snažím porovnat mou zjištěné výsledky s výsledky z jiných zemí, konkrétně z Iráku, Řecka a Číny. Bohužel jsem nenašla studii podobného rozsahu z jiné nemocnice v České republice pro lepší porovnání. Výsledkem mé práce je celkový počet karbapenemáz u vyšetřených pacientů v období od 1. 6. 2021 do 31. 12. 2022 a jejich procentuální feno-typové zastoupení v Nemocnici na Homolce. Fenotypové zastoupení z mého sledované-ho souboru se lišilo od celorepublikových dat z Národní referenční laboratoře (NRL), což mohlo být způsobeno nedostatečnou velikostí mého souboru a malým počtem pozitiv-ních vzorků. Nejvíce zastoupenou karbapenemázou v Nemocnici na Homolce ve sledo-vaném období byla OXA-48 like, kdežto z průzkumu NRL v roce 2022 byla nejčastěji zastoupena karbapenemáza typu NDM. |
| Abstract in different language: | This work focuses on antibiotics, specifically carbapenems and their resistance in Ente-robacteriaceae. I try to briefly describe the history of antibiotics and specifically car-bapenems, their mechanism of action, mechanisms of resistance and I try to characterize clinically important genera of the Enterobacteriaceae family, specifically Escherichia coli, Klebsiella spp. and Enterobacter spp. I especially focus on their laboratory diagnosis and associated antibiotic susceptibility testing. In the practical part I deal with the analy-sis of data from the newly introduced laboratory detection of carbapenemases in Entero-bacteriaceae at the Hospital na Homolce, which was the main aim of this thesis. On the basis of the objectives I set research questions, which I managed to answer in the practi-cal part of the thesis. In the discussion I try to compare my findings with results from other countries, namely Iraq, Greece and China. Unfortunately, I did not find a study of similar scope from another hospital in the Czech Republic for a better comparison. The result of my work is the total number of carbapenemases in the examined patients in the period from June 1, 2021 to December 31, 2022 and their percentage phenotypic representation in Hospital na Homolce. The phenotypic representation from my study sample differed from the national data from the National Reference Laboratory (NRL), which could be due to the insufficient size of my dataset and the small number of positive samples. OXA-48 like was the most represented car-bapenemase at the Hospital na Homolce during the study period, whereas NDM was the most repre-sented carbapenemase from the NRL survey in 2022. |
